Agenda Item Request Form 

- This form must be returned to Peru City Hall on the Thursday before the regular city council meeting at noon, or noon on the second business day before a special council meeting.

Building Permit Requirements


Opening Utility Account Form

- When submitting this form, you must print it out and sign it before sending it. You can send it to City Hall via mail, our City Dropbox outside City Hall, or bringing it directly to the Clerk's office. We do not accept emailed Utility Forms.
 

- You must also include a copy of any state-issued photo I.D. This includes driver's license, passport, or state I.D. card. Please call our office for more information.
 

- You must also pay a $100 deposit to the City before any account is opened. This must be in the form of a cash or check along with this form.​
